2.What is a Full Medical Record?
Your Full Medical Record includes important clinical information about you. This is mandatory for us to see when undertaking a Tfl PCO medical assessment.Please see the PCO webpage to find more about how you can request for this with your GP.
3.What if I forget to bring my Full Medical Record?
Please note that we will not be able to conduct the medical without your full medical records. There will NOT be a refund as you will be charged for the doctor's time.
4.Where can I obtain a D2 form?
We cannot issue D2 forms, but you can order your pack via the link below. Please complete the D2 form and access the DVLA website for further information should you require it.Â
5.What do I need to bring to the appointment?

6.Can I still drive if I am close to the expiry date?
If you have sent the form close to the expiry date and havenât received your licence back, you are still legally entitled to drive under Section 88 of the Road Traffic Act 1988.
7.What if I need to re-arrange my appointment time/location?
You can rearrange your appointment up to 12 hours prior to your appointment, by accessing the confirmation email you would have received after booking.
8.What if I turn up late for my appointment?
We recommend you attend at least ten minutes early to your appointment. Our lists are normally full, and we will not be able to delay the entire clinic.
If you are more than ten minutes late you are required to email and re-arrange.
9.What if I need to cancel my appointment?
You can cancel your appointment up to 12 hours prior to your appointment, by accessing the confirmation email you would have received after booking your medical.
Please be aware that we will not be able to provide you with a refund, however you will be able to re-arrange your appointment, at no additional cost. Please see below FAQ.
10.How long should the appointment last?
The one-to-one examination will take approximately 10 minutes.
11.What if I fail the eye test?
We will signpost you to the nearest optician for further assessment. This will be required prior to us fully completing the driver medical assessment.
12.How long is my D4 medical form valid for?
A D4 medical is valid for four months from the date of completion.
13.What if I develop a new medical condition?
It is your responsibility to promptly inform the DVLA of any changes to your health at all times. Please see the link below for specialised details.
14.What payment methods are accepted?
We accept only card payments and you will be required to pay at the time of booking via our secure payment system online.
