D4 Medical
A D4 medical exam by a GMC registered physician is mandatory, regardless of whether you are applying for a new group 2 license or simply to renew your existing one.
Who requires a Group 2 licence?
Drivers of HGV/LGV/PCV, Minibus and minibuses must have a Group 2 license. This includes the following vehicle types:
- Lorry
- Bus
- Coach
- Ambulance (C1 Medical)
- Minibus (D1 Medical)
In the following situations, you will also need the medical examination report (D4):
- This is your first request for a provisional license of group 2.
- Category C1 (7.5t), C (rigid) or D1 (minibus), D (bus).
- You must renew your group 2 license every five years if you are over 45.
- You must renew your group 2 license every year if you are over 65.
The time between your medical exam and applying for your Licence (or renewing your Licence) must not exceed four months.
Any changes in your health must be reported to the DVLA immediately.
Documents and items, you need to bring
Photo ID
Your Driving License or Passport
Your driving glasses or contact lenses
and your most recent prescription
Details of your medical conditions
Details of your current medications and doses
a repeat prescription is preferred
What does the medical examination involve ?

- You must declare that you have given correct information to your doctor and are responsible for the contents.
- You are legally responsible to inform the DVLA if you become aware of any changes in your health.
