D4 Medical

A D4 medical exam by a GMC registered physician is mandatory, regardless of whether you are applying for a new group 2 license or simply to renew your existing one.

Who requires a Group 2 licence?

Drivers of HGV/LGV/PCV, Minibus and minibuses must have a Group 2 license. This includes the following vehicle types:

  • Lorry
  • Bus
  • Coach
  • Ambulance (C1 Medical)
  • Minibus (D1 Medical)

In the following situations, you will also need the medical examination report (D4):

  • This is your first request for a provisional license of group 2.
    • Category C1 (7.5t), C (rigid) or D1 (minibus), D (bus).
  • You must renew your group 2 license every five years if you are over 45.
  • You must renew your group 2 license every year if you are over 65.

The time between your medical exam and applying for your Licence (or renewing your Licence) must not exceed four months.

Any changes in your health must be reported to the DVLA immediately.

What does the medical examination involve ?

Arrive a the location with your documents including the DVLA D4 form.

Your 10-minute appointment will comprise of: an assessment of your physical health, a blood-pressure check and an eye test.
